Support Bootswatch in example applications #254

Offer a page in the example applications that support Bootswatch. Can be implemented with a radio button offering:

default (no Bootswatch)
app.config['BOOTSTRAP_BOOTSWATCH_THEME'] = 'flatly'
app.config['BOOTSTRAP_BOOTSWATCH_THEME'] = 'darkly'
