[Bug] PDF Import Naming

Each of these Input Boxes has an "Input Name" in the PDF, but it's not being used, they are just called default text field"

In addition to the import not recognising radio boxes (it recognises check boxes)
It also skips input boxes if there is any default text.

Sure, it may look ugly in places. I've removed company branding, etc..

Perhaps it skips Radio Buttons for the same reason it skips Input Boxes. The Radio Buttons have Labels, and some of the input boxes have default text.
